If you submit a referral form via this website, you may be providing Protected Health Information (PHI). We treat all such information with the highest level of security in compliance with HIPAA and HITECH Act regulations.

We use the information we collect to provide CalAIM Enhanced Care Management (ECM) and Community Supports services. Specifically, we may use your information to:

  • Process referrals for ECM and Housing services.

  • Verify eligibility with Medi-Cal Managed Care Plans (e.g., L.A. Care, Health Net, Molina).

  • Coordinate care with your doctors, housing navigators, and social workers.

  • Send you administrative emails regarding your account or services.

  • Respond to your inquiries and offer support.

3. Disclosure of Your Information

We do not sell, trade, or rent your personal identification information to others. We may share information we have collected about you in certain situations:

  • By Law or to Protect Rights: If we believe the release of information is necessary to respond to legal process, to investigate or remedy potential violations of our policies, or to protect the rights, property, and safety of others.

  • Service Providers: We may share your information with third-party vendors (such as our secure email hosting or CRM providers) who perform services for us or on our behalf and require access to such information to do that work. All such partners are bound by Business Associate Agreements (BAA) where required by HIPAA.

  • CalAIM Coordination: As an ECM provider, we are required to share data with the California Department of Health Care Services (DHCS) and Managed Care Plans to facilitate your care.
