Process referrals for ECM and Housing services.
Verify eligibility with Medi-Cal Managed Care Plans (e.g., L.A. Care, Health Net, Molina).
Coordinate care with your doctors, housing navigators, and social workers.
Send you administrative emails regarding your account or services.

CalAIM Coordination: As an ECM provider, we are required to share data with the California Department of Health Care Services (DHCS) and Managed Care Plans to facilitate your care.
