Risen Health Partners is preparing to provide field-based Enhanced Care Management and housing-related Community Supports for eligible Medi-Cal members in Los Angeles County. Services are subject to eligibility, Medi-Cal Managed Care Plan coverage, referral, and authorization requirements.
Housing Deposits
We may help eligible members access authorized support for security deposits, utility setup, and essential household items when approved through the member’s Medi-Cal Managed Care Plan.
Tenancy Sustaining Services
Once you move in, we help you understand your lease, resolve issues with landlords, and prevent eviction.
Short-Term Post-Hospitalization
A safe place to recover and heal after you leave the hospital or ER, while we work on your long-term plan.
Enhanced Care Management (ECM)
We help coordinate medication review, appointment follow-up, transportation, referrals, and communication among your care team.
Housing Transition & Navigation
We help you search for housing, fill out rental applications, and talk to landlords to get you approved.
Transitional Rent
Transitional Rent readiness planning for eligible members, subject to Medi-Cal Managed Care Plan coverage, authorization, and implementation requirements.
