At Risen Health Partners, our mission is to bridge the gap between healthcare and housing for Los Angeles’ most vulnerable populations. We are dedicated to providing dignified, Whole-Person Care that addresses the root causes of poor health.
By integrating Enhanced Care Management (ECM) with housing support, we empower Medi-Cal members to regain their stability, improve their health outcomes, and rebuild their lives with confidence.

We envision a Los Angeles where health equity is a reality, and no individual has to choose between medical care and a safe place to sleep.
We strive to create a system where Social Determinants of Health—like housing and food security—are treated with the same urgency as medical conditions. Our goal is to be the trusted partner for Health Plans and the community, ensuring that every Angeleno has access to the resources they need to thrive.
