“Community Supports” are special services offered by CalAIM to help members with social needs that affect their health. Risen Health Partners specializes in Housing Services.
We understand that you cannot be healthy if you don’t have a safe place to sleep. Our team works directly with landlords, shelters, and hospitals to bridge the gap between healthcare and housing.
Official Term: Housing Transition Navigation Services Description: We help you search for available apartments, complete rental applications, and negotiate with landlords to get you approved. We are by your side until you sign the lease.
Official Term: Housing Deposits Description: Don't let startup costs stop you. We can help pay for your security deposit, first month's utilities, and essential household items (like a bed or heater) to help you move in.
Official Term: Housing Tenancy & Sustaining Services Description: Once you have a home, we help you keep it. Our team supports you in understanding your lease, resolving disputes with landlords, and connecting you to resources to prevent eviction.
Services
These services are free for eligible Medi-Cal members in Los Angeles County who meet specific criteria, such as:
Experiencing Homelessness: Currently unhoused or sleeping in shelters/vehicles.
At Risk of Homelessness: Facing eviction or losing housing soon.
High Utilizers: Frequent visits to the ER or hospital admissions.
Transitioning to the Community: Leaving a nursing facility or incarceration.
