Tenancy Sustaining Services
Once you move in, we help you understand your lease, resolve issues with landlords, and prevent eviction.
Short-Term Post-Hospitalization
A safe place to recover and heal after you leave the hospital or ER, while we work on your long-term plan.
Enhanced Care Management (ECM)
Your dedicated guide to help you schedule doctors’ appointments, find transportation, and manage your medications.
Housing Transition & Navigation
We help you search for housing, fill out rental applications, and talk to landlords to get you approved.
Transitional Rent
Assistance with rent payments for up to 6 months to help you get back on your feet (for eligible members).
